<p>The Hpv Vaccine: Why It Won&#8217;t Promote Sexual Promiscuity</p>
<p>Ever since the FDA approved the HPV vaccine two years ago, its introduction into the health community has been embroiled in a medical, social,<span id="more-410"></span> cultural and political controversy. At first glance, the debut of a vaccine to fight most cervical cancers looks like an exciting advance in the history of women&#8217;s health. However, many people are unable to separate the ethics from the basic scientific facts: the virus is transmitted through sexual contact and hence the vaccine is recommended for 11 or 12-year-old girls, before they first become sexually active. The perceived possibility that administering such a vaccine to prevent a sexually transmitted disease could promote sexual promiscuity has trumped all other ethical debates, leading some people to cast its benefits aside. As a mother, I can understand why many parents are concerned about this issue and put off vaccinating their daughters. However, in addition to being a mother, I am a physician, and I therefore believe that by putting off vaccination, we effectively strip our daughters from their chances of protecting themselves from the two major cancer causing HPV types.</p>
<p>According to a Newsweek article from February 25, 2008, only two out of every 10 women in the U.S. in the approved age group have received the vaccine so far. In another new survey of almost 10,000 parents, only 49 percent said they would get their daughters vaccinated with the HPV vaccine at the recommended age of 9 to 12 years. Even though 68 percent planned to vaccinate their daughters at 13 to 15 years of age, and 86 percent said they would vaccinate at 16 to 18 years of age, they might be too late &#8212; because the vaccine can only prevent and not cure HPV. The National Survey of Family Growth (NSFG), an agency that collects data on the sexual behavior of American teenagers, reports that teenage girls are already sexually active: 13 percent of them by the time they are 15, 43 percent of them by age 17, and 70 percent of them by age 19. Of the sexually active 13-21 year age group, 70 percent show evidence of HPV infections within a few to several months of when they start having sex &#8212; most of which could easily be prevented by vaccinating preadolescent girls. In addition, many parents are uncomfortable talking to their preadolescents about sexual issues; meaning uneduc<br />

ated teens could make problematic sexual choices not only without realizing the consequences of their actions, but also without the protection from HPV that the vaccine provides.</p>
<p>Again, bear in mind that the vaccine is preventive &#8212; it is only effective if administered prior to exposure to the virus. Therefore, the vaccine will not treat the existing infections. This is why it&#8217;s so crucial to administer the vaccine when girls are young, before any chance of sexual activity, including sexual contact without intercourse. In addition, there are several other scientific reasons why the vaccine is recommended in the 11-12 year old groups. Unfortunately, it is beyond the scope of this article to discuss those facts.</p>
<p>But, won&#8217;t vaccinating young girls against a STD cause them to become less sexually inhibited and more promiscuous by giving them a license not to worry about the consequences of sex? Won&#8217;t it eliminate a deterrent to teenage sex and therefore encourage it? Highly unlikely: because teens rarely factor the possibility of getting HPV into their decisions to have sex.</p>
<p>Current research in adolescent sexual behavior shows that not one, but a variety of factors influence teenage sexual behavior, such as their socio-economic status, their moral and religious values, their sexual attitudes, and the influence of family, friends, and the society they live in. Therefore, it is highly unlikely that a vaccine against a single kind of STD would play a more significant role in adolescent decision making than these factors paired with their responsibility, good judgment, and the boundaries of accepted sexual behavior.</p>
<p>According to NSFG, the top two factors influencing teenagers who choose to remain virgins are fear of pregnancy (94 percent) and contracting HIV/AIDS (92 percent). Since knowledge and awareness of HPV in this age group is poor to begin with, teens will not view this vaccine as a reason to become sexually active at an age sooner than intended. And parents who worry about the vaccine causing promiscuity should remind themselves of the two most dangerous and important factors that influence teenagers to become sexually active and engage in high-risk behaviors: peer pressure and the use of alcohol and/or drugs.</p>
<p>Parents who worry about their teens becoming promiscuous can also focus on a crucial method to keep them from becoming sexually active too soon: raising their awareness. Research has shown that parental guidance is one of the most important factors in delaying teenage sexual activity &#8212; teenagers who do not have involved parents are most likely to engage in high-risk behaviors. The HPV vaccine can actually help parents guide their teens&#8217; sexual decisions by giving them a window of opportunity to talk about the topic, and to get health care providers involved in the discussion. There is no reason why parents cannot, without any hypocrisy, inform their daughters that the vaccine is not a green light for them to immediately have sex. Rather, it is a strong message that you, their parents, care about their health, and that it&#8217;s important to take proactive steps to become a healthy adult. Think of it this way: Getting a car with an airbag, traction control and anti-lock brakes means that the car is safer in the long-run, but these features aren&#8217;t a reason to drive recklessly &#8212; accidents are still possible. No matter what, the vaccine will not, cannot and should not take the place of good, solid parental guidance.</p>
<p>The bottom line is we as we as parents want to do what is best for our children. We all want to teach them well while they are still in our protective cocoon so that we can trust them with their own judgment when they become independent. If they always followed our best wishes, we wouldn&#8217;t need to worry about administering the vaccine at all. However, even teens with the best judgment can face peer pressure and fall victim to bad decisions or other reasons that may cause them to engage in risky sexual behavior, and this makes the ca<br />

se for vaccinating all the more compelling.</p>

<p>Genital warts can be chemically burned or frozen with liquid nitrogen or cryosurgery. Although these techniques for freezing the infection are effective, they can cause a temporarily annoying blistering of tissue. Some cases of genital wart infection are treated by applying caustic chemicals to the infected area. </p>
<p>More than one application is usually needed. Laser treatment might be recommended for recurrent warts that have been resistant to other treatment methods. Most of these treatments must be repeated several times, sometimes over several months. All methods of treatment have high failure rates. Therefore, reexaminations sometimes are recommended, even after the warts seem to go away. Genital warts cannot be effectively treated with any over-the-counter preparations.</p>
<p>You may need longer treatment if you have HIV. The warts may increase in size and number more quickly. Tell your clinician if you are HIV positive.</p>
<p>Warts may return, even after treatment, this is because the virus stays in your skin once you are infected. You can pass the virus to your sex partners during vaginal or anal sex, even when you don&#8217;t have warts you can see.</p>
<p>It follows from the fact that warts tend to retrun that there is no cure for the virus that causes genital warts. Warts can be treated so that they can no longer be seen, and that is about the limit of it.</p>
<p>Neverthless, there are several effective treatments for genital warts, including application of chemicals, freezing with liquid nitrogen, or self-treatment with a prescription medication cream. No matter what method is used, most warts will need more than one treatment. Choice of treatment method depends on patient preference, convenience and availability. Discussion with a health care provider can help determine the best method of treatment for an individual. <p>INTRODUCTION: Cervical cancer is a malignancy of the cervix uteri or cervical area. it is a carcinoma, typically made up of squamous cells, and is akin in<span id="more-408"></span> some respects to squamous cell cancers of the head, neck and anus. These malignancies and cervical pre-cancers are classified by how they appear under a microscope.</p>
<p>In industrialized countries, the widespread use of cervical screening programs has reduced the cases of invasive cancer of the cervix by 50% or more. Human papillomavirus (HPV) infection is a necessary factor in the development of almost all cases of cervical cancer.</p>
<p>RISK: The most important risk factor in the development of cervical cancer is infection with a high-risk strain of human papillomavirus. Women who have many sexual partners (or who have sex with men or women who have had many partners) have a greater risk. The American Cancer Society provides the following list of risk factors for cervical cancer: human papillomavirus (HPV) infection, smoking, HIV infection, chlamydia infection, dietary factors, hormonal contraception, multiple pregnancies, exposure to the hormonal drug diethylstilbestrol (DES) and a family history of the disease.</p>
<p>Despite the development of an HPV innoculation, some researchers say that routine neonatal male circumcision is an acceptable way to decrease the risk of cervical cancer in their future female sexual partners. However, in men with low-risk sexual behaviour and monogamous female partners, circumcision makes no difference to the chances of developing the disease.</p>
<p>But having a risk factor, or even a number of them, does not mean that you will develop the disease. Several risk factors increase your chance of getting cervical cancer. Even though these risk factors increase the odds of developing the disease, many women with these risks do NOT develop this malignancy. When a woman develops the cancer or pre-cancerous changes, it sometimes may not be possible to say with certainty that a particular risk factor cause the disease.</p>
<p>when considering risk factors, it helps to focus on those that you can change or avoid (like smoking or HPV infection), rather than those that you cannot (such as your age and family history).</p>

Although almost all cervical cancers are either squamous cellcarcinomas or adenocarcinomas, other types of cancer also can start in the cervix. These other types, such as melanoma, sarcoma, and lymphoma, occur more commonly in other parts of the body.</p>
<p>Certain types of HPV are called &#8220;high-risk&#8221; types because they are often the cause of cancer of the cervix. More than 250 types of HPV are acknowledged to exist (some sources indicate more than 200 subtypes). Together, HPV types 16 and 18 currently cause about 70% of cases. HPV types 6 and 11 cause about 90% of genital wart cases. Different types of HPVs cause warts on different parts of the body. Some types cause common warts on the hands and feet. Other types tend to cause warts on the lips or tongue. Still other types of HPV may cause warts on or around the female and male genital organs and in the anal area.</p>
<p>TREATMENT: Treatment consists of surgery (including local excision) in the beginning stages and chemotherapy and radiotherapy in advanced stages of the disease. On June 15, 2006, the Food and Drug Administration approved the use of a combination of 2 chemotherapy drugs, hycamtin and cisplatin for women with late-stage (IVB) cervical malignancy.</p>
<p>Because of treatment, the five year relative survival rate for the earliest stage of invasive cancer of the cervix is 92%, and the overall (all stages combined) five year survival rate is about 72%. These statistics may be improved when applied to women newly diagnosed, keeping in mind that these outcome may be partially based on the state of treatment five years ago when the women studied were first diagnosed. With treatment, 80 to 90 percent of women with stage I cancer and 50 to 65% of those with stage II cancer are alive 5 years after diagnosis.</p>
<p>As the cancer metastasizes to other areas of the body, prognosis drops considerably because treatment of local lesions is often more effective than total body treatments such as chemotherapy. 35% of patients with invasive cancer of the cervix have persistent or recurrent disease after treatment. In the majority of cases however the body&#8217;s immune system fights off the virus, and the infection goes away without any form of treatment.</p>
<p>CONCLUSION: The early stages of cervical cancer may be completely asymptomatic. Types 16 and 18 are generally acknowledged to cause about 70% of cases. Together with type 31, they are the prime risk factors for cancer of the cervix.</p>
<p>The medically accepted paradigm, officially recognized by the American Cancer Society and other organizations, is that a patient must have been infected with HPV to develop this form of cancer, and is therefore viewed as a sexually transmitted disease, however most women infected with high risk human papilloma virus will not develop the disease. There has not been any definitive evidence to support the claim that male circumcision prevents the disease, although some researchers say there is compelling epidemiological proof that men who have been circumcised are less likely to be infected with HPV.</p>
<p>While the pap smear is an effective screening test, confirmation of the diagnosis of cervical cancer or pre-cancer requires a biopsy of the cervix. According to the US National Cancer Institute&#8217;s 2005 Health Information National Trends survey, only 40% of American women surveyed had heard of human papillomavirus (HPV) infection and only 20% had heard of its link to cancer of the cervix.</p>
<p>In 2008 an estimated 3,870 women in the United States will die of cancer of the cervix, and<br />

around 11,000 new cases are expected to be diagnosed. The American Cancer Society recommends that screening should begin approximately 3 years after the onset of vaginal intercourse and/or no later than twenty-one years of age.</p>
<p>The HPV test is a newer technique for cervical cancer triage which detects the presence of human papillomavirus infection in the cervix. Since more than 99 percent of invasive cervical cancers worldwide contain HPV, some researchers recommend that HPV testing be done together with routine cervical screening.Human papillomavirus testing can decrease the incidence of grade 2 or 3 cervical intraepithelial neoplasia or cancer of the cervix detected by subsequent screening tests among women 32-38 years old according to a randomized controlled trial.</p>
<p>Figures suggest that cervical screening is saving 5,000 lives each year in the UK alone by preventing this disease. Worldwide, cancer of the cervix is the fifth most deadly cancer in women.</p>

<p>Common warts are ones that appear on hands and fingers while plantar warts are one that occurs on the soles of our feet. Common skin warts are either yellow, light<span id="more-405"></span> gray, brown or gray-black, and are firm to the touch. They normally appear on the back of your hands, or near the fingernails.</p>
<p>Genital warts and HPV infection are transmitted primarily by sexual intimacy, and the risk of infection increases as the number of sexual partners increase. These warts are the most contagious, while common, flat, and plantar (foot) warts are much less likely to spread from person to person. </p>
<p>A number of problems can be caused by genital warts may during pregnancy. But before you get too alarmed, do realise that the problem is usually simply that if the warts are in the vagina, they can make the vagina less elastic and cause obstruction during delivery.</p>
<p>Warts of the genitals look like flesh colored masses of tissue. In some cases they may appear as bumps. Warts from sexual transmission can be raised or flat, small or large. Sometimes they&#8217;re clustered together in a cauliflower-like shape. These warts can get large during pregnancy, making urination difficult.</p>
<p>Warts on our private parts are the most common symptoms of HPV infection. However, most HPV infections do not lead to either warts or cancer; most infections go away by themselves, without causing any symptoms. </p>
<p>Genitally transferred warts may or may not show symptoms. If they do, the warts may appear as tiny cauliflower-shaped lesions that may be raised or flat. Genital warts do not affect the baby during pregnancy.</p>
<p>Genital Herpes (wart infection) sometimes occurs as a cluster of (often painful) fluid filled &#8220;blisters&#8221; that appear on a background of skin that is reddish or slightly inflamed. Fresh outbreaks of herpes after their initial outbreak is (we have been told) common when infected with HSV (type 1or 2) and may be associated with the menstrual cycle, emotional stress, illness (especially with fever), sexual intercourse, surgery, and certain medications&#8230; The medical profession calls these trigger factors. </p>
<p>Human Papillomavirus (HPV), is the correct term for warts of the genitalia and the course of the infection sometimes is not straightforward. Because, like it or not, they are small fleshly skin tumors of the<br />

 genital and perianal area caused most often by the most common sexually transmitted virus, HPV 6 and 11. As we have already begun to describe Human Papilloma Virus spreads by skin to skin contact during vaginal, oral and anal sex and is estimated to affect 1% of the sexually active population aged between 16 and 50.</p>
<p>Warts and corns can be treated at home when accessible on the body, and not in a sensitive location. The idea is to remove thickened, diseased skin without damaging surrounding healthy tissue, and by so doing help the body to win over on the virus by removin the seat of the infection.</p>
<p>In the case of genital wart problems though home care is not really satisfactory, and you should obtain medical advice from the start and as soon as you see them.</p>
<p>Warts will not normally leave scars, though some of the more aggressive wart therapies might. The fact is though that you can re-infect yourself again by touching the wart and then another part of your body.</p>

<p>Warts may be frozen with various commercial freezing probes or with liquid nitrogen sprayed on or applied with a cotton swab. Cryotherapy is often used for plantar warts and warts under the fingernails. </p>
<p>Physicians themselves have said in the past that they do not know how to get rid of genital warts, making it difficult to keep coming back to get genital warts removed. Costly surgical procedures often leave scars, are painful, and require a significant amount of healing time. </p>
<p>Salicylic acid is the chemical most often found in over the counter products. Another similar acting chemical is retinoic acid. Salicylic acid is commonly used to treat normal warts, and some believe that it would help their genital wart condition, but they could be no more wrong. Using home cures such as salicylic acid can only damage your body.</p>

<p>Genital warts typically affect the moist tissue areas of the genital area. They typically appear as small, flesh-colored bumps that can change to a cauliflower-like appearance <a href="http://online.pillslib.com/mysoline/" target="_blank" rel="nofollow">if</a> they are left untreated. Genital warts can range in size from one to two millimeters in diameter or to large sizes if they multiply into large clusters. In some cases genital warts have been known to disappear on their own.</p>

For men genital warts can be found on the tip or shaft of the penis, the scrotum or the anus. <p>Human are infected by over one hundred types of papillomaviruses. More than forty of these will infect the anogenital tract of both men and women, resulting in genital warts. There is a subgroups of these viruses that can cause precancerous changes in the uterine cervix and cause uterine and cervical cancer. HPV has also been associated with the development of other anogenital cancers in women.
